How Do Open and Closed Rhinoplasty Differ?

Nose surgery—medically known as rhinoplasty—at the Philadelphia area’s Cross Medical Group is one of the most popular procedures chosen by patients each year. As a plastic surgery option, it remains at the top of many people’s lists because a little change can make a big difference to someone’s appearance.

Wondering why? Consider how prominent the nose is. It figuratively and literally sticks out, often dominating the face for better or for worse. A nose that appears outsized or out of proportion can draw attention and even make other features appear imbalanced.

With this in mind, plastic surgeons have developed a range of techniques to shape nasal tissue, giving them the ability to reduce the nose’s size, alter the shape of the nostrils, smooth humps and bumps, position and direct the nasal tip, increase or decrease its width, and make any number of adjustments so that it better “fits” with a patient’s personal aesthetic and desire for their appearance.

When it comes to the nose surgery itself, there are two primary variations: open and closed. The names of these techniques are fairly self-explanatory, but here’s a closer look at what the techniques entail.

Open Rhinoplasty

Some nasal surgeries benefit most from the open approach, which involves Dr. Kevin Cross or Dr. Jackie Lyons making incisions that allow them to physically lift off the skin the covers the nose, exposing the structures below. This gives them a clear view of the internal features they are working with, as well as provides access for excision, shaping, and other necessary steps. Once the procedure is done, the skin is draped back into place and securely sutured. The result is a newly shaped nose and a small, barely noticeable scar on the thin strip of tissue separating the nostrils, known as the columella.

Closed Rhinoplasty

If direct visualization of the inside of the nose isn’t needed, Dr. Cross or Dr. Lyons may opt for a closed rhinoplasty, which leaves the surface skin intact. All access is achieved through incisions made internally. This understandably limits the surgeon’s view, but also avoids the creation of any visible scar. For the right patient, this can be a highly effective strategy.

Which Is Better: Open or Closed?
